Rates
An initial evaluation session is $200.00. A 60 minute session is $160.00.
Insurance is Accepted. I am in-network with the following insurances:
Blue Cross Blue Shield PPO
Aetna
Cigna
Medicare
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits? Specify that it will be in an "office setting."
- What is my deductible and has it been met? Does my deductible apply to my mental health benefits? Is there an additional deductible for mental health benefits?
- How many sessions per year does my health insurance cover?
- How much does my plan cover for an out-of-network provider (if not one of the insurances identified above)?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Is pre-authorization required from the psychologist? (I am happy to complete forms for you)
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, you will be required to pay a $85.00 fee that is not covered by insurance. If you call with less than a 24 hour notice, the late cancellation fee is $75.00 and is also not covered by insurance.
